1. I would like to participate in the Students of GVSU project

2. I understand that my participation is voluntary and that I can withdraw from the study at any time for any reason

3. I understand that I can share as little or as much personal information as I choose in my answers to the topic word

4. I understand that my participation is anonymous, and I do not have to give any other personal information other than my response to the topic word

5. I understand that I will not publicly be identified in this project and that only my answer will be printed

6. I understand that I can ask that my response be removed from the project even after it's been submitted if I decide to withdraw my participation
